Davis Vision Discontinuation Letter

 

This letter was sent out to all of our current and past
patients that have Davis Vision insurance




February 13, 2007


Dear Valued Patient:

It is with regret we must inform you as of May 1, 2007, we will no longer be participating with the Davis Vision Plan.

We not only have had numerous problems with Davis Vision in receiving our optical orders in a timely fashion, but also with the correctness of those orders. We do not believe this serves the best interest of our patients, nor does it reflect the philosophy of Total Vision.

We are more than happy to offer you a 10 percent savings when you pay by cash, check or debit card. We will also provide you with an itemized bill which you, in turn, can file with your plan for member reimbursement.

We can, of course, continue to treat medical issues such as infections, glaucoma, and eye injuries or emergencies. We will bill those services to your medical insurance (Blue Cross, Principal, Primary Health, etc.)

We are sorry for any inconvenience this may cause and appreciate your business. We hope we can continue serving you as your eye care specialist. Our goal remains Keeping You Seeing As Long As You Live.
