We're Still Your Eyecare Provider!

In response to requests from our valued patients who have Davis Vision, we'd like to present a way to continue your eyecare at Total Vision. The following steps below make it quick and easy to find out how to get reimbursed by Davis. (You may want to print out these instructions.)

• Before making your appointment with us, please verify your benefit eligibility by clicking on www.davisvision.com.
• Click on the green "Member Login" button on the right side of the web page.
• Enter your Login name (usually the social security number or the employee ID number of the primary insured) and your password (usually the first five letters of your last name)
• Click on "Eligibility" to be sure you're eligible for the services you want.
• Then click on "Information and Forms", then on "Out of Network"

This will give you the forms you will need to file with Davis Vision for reimbursement. Bring these forms to our office, as there is a portion that needs to be done by us.

Since this is a reimbursement situation, payment is due at time of service. We look forward to seeing you soon.
